1. To request Court Supervision from the State's Attorney's Office, fill out an Online Court Supervision Request.

2. Do not pay online until you have been notified of your qualification and the amount due.

3. After such notification and after the Circuit Clerk's Office has received your Signed Guilty Plea (blue copy of ticket).

If you have already paid your ticket on line, or have sent in the blue copy of your ticket and/or paid the ticket fine amount:

You must file a Motion to Vacate Guilty Plea with the Circuit Clerk's Office and pay an additional fee before continuing with your Request for an Order of Supervision.

This fee is set by State Statute:

• Less  than 30 days since conviction date: $20.00 to file motion.
• More than 30 days since conviction date: $40.00 to file motion.
